美国促医保支出上涨一大因素：医生正在其诊所里做更多测试

住在纽约州伍德布里奇（Woodbridge）的博伊德是一名家庭医生，近年来他开始对一种少见的神经系统疾病频频为患者进行测试。而他采用的装置是通过人们对低电压电流的反应来诊断其神经是否存在损伤。在2014年，他因此从医疗保险项目获得了10万5905美元的收入。



据《华尔街日报》报道，在最近几年类似博伊德这样的家庭诊所测试在全国范围内呈上升趋势，法律也允许医生在他们的诊所内使用合格的设备为客户进行测试，同时，这些诊所也从医疗保险中获得了更多的利润。据最新数据显示，联邦保险计划为老年人和残疾人因此支付了1670万美元，与两年前相比增加了10倍。

《华尔街日报》最近发布的一份医疗保险账单数据分析显示，2012至2014年间导致医保支出增长最快的10大因素中，与新设备使用相关的就有4个，而这4个因素导致增加的医保费用达到1亿3500万美元，而在每一种情况下，医生在自己诊所内使用设备产生的费用涨幅要比其它情况大得多。

尽管与医疗保险6000亿美元的年度预算相比，这些增长的数目并不显眼，但对于政府需要补助的无数医疗单位和项目而言，任何部门开支的飙升都会明显导致国家健康成本上涨。数据显示，2014年政府医保代理商向医生丶实验室和其他医疗服务提供商支付的服务项目至少有1万1448个。而医疗保险覆盖新医疗服务项目的做法也让投资于新设备的医生看到了测试的商机。

而随着医疗技术的进步，最近几年如遗传测试丶汗液测试等一些新测试方式不断出现，在医保覆盖的利好条件下，自然会导致这些测试服务出现增长，医生们开始主动采用一些更好的技术设备。对此，前医疗保险的官员和专家认为，随着科学进步发展，对医疗供应商的财务激励措施可大力促进对新技术的采用。

当加利福尼亚大学心脏病专家丶旧金山医疗保险咨询委员会主席雷德保（Rita Redberg）在探讨新技术的研讨会上指出，他们并不否认新技术带来的好处，但目前需要保持适度和平衡。

联邦医保机构发言人奥尔布莱特（Aaron Albright)也表示，该机构正在密切监控各项医疗服务的支出，他称，该机构可能会正确处理，但也可能会因为错估出现一些可能的与新疗法相关的不当付款。

另外，泪液测试也是目前医生诊所内非常普遍的一种新技术测试，营业额增长迅速。该测试始于2012年，是一家名为tearlab的公司最先向眼科医生诊所进行了推荐，该测试是主要是帮助医生诊断病人的眼睛是否有干眼症。目前，约3000家医疗机构其中大多是眼科医生诊所和验光师在使用这一测试，2014年该项测试服务花费达到1480万美元，大幅高于2012年该测试刚投入市场时的175万美元。

此外，汗液测试也比较流行，开始提及的博伊德医生就在引进这一新技术测试。该技术可以对糖尿病等疾病进行测试。博伊德称，当神经受到糖尿病或其它疾病损害时，它可能会导致包括疼痛等问题，也可能会引发潜在的致命性并发症。如果手和脚不正常产生汗液，这可能是一种神经损伤的迹象。

此外，市场上出现的新技术的是还有EBT癌症测试丶专门监测糖尿病的SUDOSCAN测试等，这些测试在近几年的开支也大幅呈现上升。

位于亚利桑那州塞多纳的美国放射肿瘤协会主席拜尔（David Beyer）认为，这些新技术测试爆炸性的增长很大程度上是出色营销的结果。
